利用Corvis ST分析中高度近视屈光手术前后各角膜生物力学参数变化的研究

  [摘要]目的利用Corvis ST 分析中高度近视患者在执行屈光手术前后各角膜生物力学的变化特征及其规律,为临床及科研提供⒖家谰荨7椒ㄑ∪2020年9月至2021年7月在佛山市第二人民医院接受屈光手术的中高度近视患者121例,均取右眼,共121眼。所有患者进行常规眼部检查,并采用Corvis ST 分别于术前及术后1个月记录每位患者的角膜生物力学相关参数。结果采用配对样本检验分析发现,手术前后中央角膜厚度(CCT)、眼内压(IOP),第一次压平时间(A1T)、长度(A1L)、速率(A1V),第二次压平时间(A2T)、长度(A2L)、速率(A2V),最大压陷峰距(PD)、最大变形幅度(DA)、最大压陷反向曲率半径(HCR)、最薄点厚度/厚度变化率(ARTh)、第一次压平时角膜硬度参数(SP-A1)的差异有统计学意义(P <0.05),角膜开始接触气流至达到最大凹面的时间(HCT)与Corvis生物力学指数(CBI)的差异无统计学意义( P >0.05)。结论中高度近视患者屈光手术后角膜生物力学参数除 HCT、CBI 无变化外,其余参数均有变化,因此这些变化参数在屈光手术中都应当作为参考。

  [Abstract] Objective To investigate the characteristics and rules of the changes of corneal biomechanical parameters of patients before and after refractive surgery for moderate and high myopia analyzed by Corvis ST, so as to provide a reference for clinical and scientific research. Methods A total of 121 patients with moderate or high myopia who underwent refractive surgery in the Second People's Hospital of Foshan from September 2020 to July 2021 were randomly selected. All the right eyes were selected, which were a total of 121 eyes. Conventional eye examinations were performed in all patients, and the corneal biomechanical parameters of each patient were recorded preoperatively and 1 month postoperatively using Corvis ST. Results Paired sample analysis showed that before and after operation, there were statistically significant differences in the central corneal thickness (CCT), intraocular pressure (IOP), time of the first applanation (A1T), length of the first applanation (A1L), velocity of the first applanation (A1V), time of the second applanation (A2T), length of the second applanation (A2L), velocity of the second applanation (A2V), maximum peak distance between both collapse (PD), maximum deformation amplitude (DA), highest concavity radius value of central concave curvature (HCR), the ambrosio relational thickness horizontal (ARTh) and stiffness parameter of the first applanation (SP-A1)(P <0.05). There were no statistically significant differences in the highest concavity time (HCT) when cornea began to contact air flow to reach the maximum concave surface and the Corvis biomechanical index (CBI)(P >0.05). Conclusion Except for HCT and CBI, the corneal biomechanical parameters of patients with moderate and high myopia after refractive surgery have changed. Therefore, these variable parameters should be used as references in refractive surgery.
